Epic and Site Glossary
Epic terms, Freeman acronyms, and the words this hub uses, in one place. Across the hub, the first mention of any of these terms on a page is marked with a dotted underline; hover or tap it for the short version.
The assistant inside Abridge. Ask clinical questions before, during, or after the visit and get answers with cited sources, or edit your draft note in plain language.
Beyond the Note →The summary of the visit for the patient: diagnoses, medications, instructions, and follow-up. Ask whether the patient wants it printed; if not, it goes to MyFreemanHealth.
Notes drafted from the recorded visit conversation instead of typed or dictated. Freeman is moving to ambient documentation across the enterprise, so every provider should build an Ambient speed button to start one.
Build an Ambient speed button →The on-call scheduling system. Schedules import into Epic every 15 minutes and sign providers into their inpatient teams automatically. Make schedule changes in Amion, not in Epic.
Patient Management for Providers →Nurse practitioners, physician assistants, CRNAs, and other licensed clinicians who practice under their own scope. This hub is written for physicians and APPs together.
Cosigning a note or order means a supervising provider reviews and signs it; the author keeps authorship. Attesting adds the supervising provider's own statement. Editing a resident's note transfers authorship, so review and cosign rather than rewrite.
EMR Etiquette: Notes →Dragon's version of a SmartPhrase. A block of text you insert by voice command. Built in Dragon, not Epic, so it works anywhere you dictate.
Dragon Quick Reference →Freeman's policy for running Haiku, Canto, and remote access on your personal phone or tablet. The BYOD and Remote Access request must be approved before the apps will connect.
Bring your own device →Epic's iPad app. Everything in Haiku plus fuller documentation: note writing, order entry, and a larger chart view.
Haiku and Secure Chat Setup →Epic's record exchange between healthcare organizations, built into Hyperspace. Outside notes, results, medications, allergies, problems, and immunizations appear in the chart without a records request, flowing both ways, automatically, for treatment. Freeman's connected organizations cover a wide radius around Joplin, including Northwest Arkansas and extending to Springfield and Kansas City, Missouri.
Care Everywhere or EpicCare Link →A standard summary of a patient's record (problems, medications, allergies, results, and encounters) that one electronic health record exports and another can read. CCDs from Meditech, AthenaFlow, Clarity, and Avatar are among the legacy data loaded into Epic.
What has converted so far →The In Basket folder for your unfinished documentation: pended notes, notes to sign or cosign, and orders to cosign.
Inpatient Quick Start Guide →The Epic activity for a patient's past encounters, notes, labs, imaging, and media, organized in tabs you can filter and bookmark.
Outpatient Quick Start Guide →The physician or APP accountable for a specialty's order set content or for an alert. Approves or rejects each Round Table comment and submits the set to builders.
If you are asked to be an Owner or Reviewer →The teams that resolve placement, insurance authorization, transport, and family barriers at discharge.
You enter your own orders. Verbal orders are for emergencies only.
Epic go-live facts →The standard five-digit codes for the services and procedures a clinician bills, such as 99204 for a new outpatient visit. In Epic you add the CPT code as a charge on the visit and associate a diagnosis with it.
The 48 hours before go-live when every admitted patient's active chart (orders, meds, allergies, care team) is moved from Meditech into Epic so care continues without a restart early Saturday morning on October 24.
Day One and Week One →Freeman-build readiness environments used under Epic-led training: DEMO for demonstrations and practice, CRE for department review of content before go-live.
Epic Environments →Walkers, oxygen, hospital beds, and similar equipment a patient needs at discharge; a common discharge barrier.
The Dragon dictation software on Freeman workstations. "DMO Local" is the desktop icon that launches it.
Dragon setup course →DN is your internal extension; DID is a full outside number. Both are assigned in Epic VoIP. Most physicians and APPs get both.
Phone numbers: DN and DID →A SmartPhrase you call by typing a period and its name, like .HPISEC for the Abridge History of Present Illness section. Nest one in your own note template to place Abridge content exactly where you want it.
Make It Write Like You →Approves new OPAs and material changes to existing OPAs. Meets monthly or every other month; votes electronically between meetings for urgent items.
OPA & Order Set Requests →The visit codes billed for seeing a patient (office, hospital, emergency department, critical care), at a level of service set by medical decision making or by time.
Shared care FAQ →One of the separate copies of Epic, each with a job: PRD is the live chart, the rest hold test or training data. The abbreviation shows on the desktop icon and title bar.
Epic Environments →Sending a controlled substance prescription to the pharmacy electronically. In Epic Hyperspace it adds a PDMP check, a DEA review window, and a two-factor signature: your Freeman password plus the Imprivata ID approval on your phone. In Haiku and Canto, the two factors are your face plus your Freeman password.
EPCS: Prescribing Controlled Substances →Epic's community site, opened from the globe icon in Hyperspace. Lets a provider upload SmartPhrases and their SmartLists from one Epic organization and download them at another. It requires an active login at both organizations, so it does not help once someone has left. Preference lists, personal order set versions, and QuickActions do not transfer.
Epic's secure web portal for working in an Epic organization's chart from a browser, with its own login. Outside clinicians and their staff use Freeman's EpicCare Link, at https://epiccare.freemanhealth.com, for care coordination, referrals, and viewing records for patients they share with us; organizations request accounts there and name their own site administrator. Freeman providers use another organization's EpicCare Link only when they need to place orders there, which is uncommon and arranged department by department. Freeman Medical Staff do not need an account on Freeman's own EpicCare Link.
Care Everywhere or EpicCare Link →The provider-facing way to tell the CMIO office about Epic workflows that work but could work better. Separate from IT service requests. Reviewed weekly; progress is posted on the Suggestion Tracker.
Epic Suggestions →Phone calls placed and received inside Haiku, Canto, and Hyperspace. Takes the place of Mobile Heartbeat calls from October 24, 2026.
Haiku and Secure Chat Setup →EpicU is Epic's training site (training.epic.com). EUT is End User Training, the dashboard that shows your registration, pre-work, and test-out status. Freeman network and .net login required.
Training and Test-Out →An experienced Epic physician from another health system on site to coach providers during go-live.
Help during go-live →The list of medications approved for use at Freeman. Adding a medication to the formulary goes to P&T on its own and does not go through Round Table. A medication has to be on formulary before it can be built into an order set.
The pharmacy rule →FHW is Freeman Hospital West, FHE is Freeman Hospital East, FNH is Freeman Neosho Hospital, and FFS is Freeman Fort Scott Hospital. Patient list names end with the abbreviation when lists at different hospitals share a name.
Patient Management for Providers →Added to the level of service when a physician and an APP share one visit. It goes on the visit whichever of the two bills it. In Epic it is the SharedCare modifier button.
Shared care FAQ →The person financially responsible for a patient's account. Billing statements and payment plans follow the guarantor, not proxy access.
Epic FAQ: pediatric MyChart →Epic's phone app for iPhone and Android. Chart review, Secure Chat, schedules, In Basket, clinical photos, dictation, and charge capture. Freeman login required.
Haiku and Secure Chat Setup →The shift-change report for the next provider, written from Patient Lists or the Handoff tab in the chart, with To Do items for whoever comes on.
Inpatient Quick Start Guide →A diagnosis coding system used by Medicare and payers to gauge patient complexity. Specific problem list diagnoses capture HCCs; vague ones do not.
EMR Etiquette →Epic's population health module. A care gap is a preventive or chronic care item that is due or overdue for a patient, shown in the visit sidebar with orders ready to place.
The full Epic desktop application on Freeman workstations. Everything else is a window into it.
Haiku and Secure Chat Setup →The section of the encounter that holds the diagnoses Abridge heard and the medications, labs, imaging, and referrals you verbalized during an ambient visit. Outpatient visits only at this time; coming to the ED and hospital later. Each diagnosis must be verified there before the note can be signed, and nothing is placed until you pick the specific orderable and sign. Often shortened to HIVR on the Epic toolbar.
Heard in Visit Recording, step by step →Epic's secure inbox for results, messages, cosigns, and refill requests. Review every working day.
EMR Etiquette: In Basket →A shared In Basket folder worked by a team rather than one person, so messages and results are covered when an individual is out.
EMR Etiquette: In Basket →Freeman's IT Service Request system at fhsitsm.saasit.com. Every order set, SmartSet, and OPA request starts here, as does the BYOD and Remote Access request.
On-Site & IT →The visit's billing level, chosen before you sign the visit, with any modifiers. An unsigned visit is not billed.
Outpatient Quick Start Guide →Highlight a word, phrase, or paragraph in an Abridge draft and see where in the recorded conversation it came from. Double-click to hear the audio.
Beyond the Note →The button that timestamps your review of allergies, medications, problems, and histories. Once per visit through the Review activity covers all of them.
EMR Etiquette →The problems addressed, the data reviewed, and the risk of the plan. With time, one of the two ways to choose an E/M level of service.
Shared care FAQ →Deciding, medication by medication, what continues, stops, or changes when a patient is admitted, transferred, or discharged.
Inpatient Quick Start Guide →The legacy EMRs Epic replaces: Meditech (inpatient), AthenaFlow (clinic), Clarity, and Avatar. All remain view-only after go-live until the legacy archive is operational, estimated April 2027. Only signed notes convert.
Sign your legacy notes →Getting to older records →Proving it is you with two factors at once: your password plus a second check. Signing in to Haiku and Canto, the second check is approving a Duo Mobile or Microsoft Authenticator prompt on your phone. Signing a controlled substance, it is Imprivata: the Imprivata ID approval in Hyperspace, your face in Haiku and Canto.
Haiku and Canto setup →A common scale for comparing opioid doses. The PDMP shows a patient's total daily MMEs so you can spot an excessive combined dose before you prescribe.
EPCS: Check the PDMP →Freeman's secure texting and calling app on hospital phones through October 23, 2026. Secure Chat and Epic VoIP take its place from October 24.
Haiku and Secure Chat Setup →Freeman's MyChart (mychart.freemanhealth.com), the patient's own portal: results, visit summaries, messages, and scheduling. Results and notes release to patients here immediately when final. Not the same as EpicCare Link, which is for providers.
Care Everywhere or EpicCare Link →Your saved Abridge settings for how a drafted note is written: which sections generate, concise or comprehensive, bulleted or paragraph, which physical exam template, plus free text instructions that apply to every future note. Set them once in the Abridge activity under the settings menu, then Preferences, then Note settings.
Advanced Ambient: your note preferences →Epic's point-and-click documentation tool for exam findings, review of systems, and procedures. Procedure notes written in NoteWriter drop charges automatically.
Important Changes with Epic →The Epic activity that shows the call schedule for any service, with who is on call now. All providers have access.
Patient Management for Providers →Epic's pop-up alert that fires on a rule, such as a drug interaction or a missing order. Formerly called a BestPractice Advisory (BPA). Requests to add, change, or silence one go through the OPA and Order Set Requests page.
OPA & Order Set Requests →A small reusable group of orders that can be placed alone or inside several order sets. A panel gets its own Round Table review when it affects more than one order set; a panel used inside a single set is reviewed with that set.
Order Set and OPA Governance →A grouped set of orders for a condition or procedure. Changes and new requests go through the OPA and Order Set Requests page.
OPA & Order Set Requests →Reviews any order set that contains medications, plus anything else the Pharmacy P&T Coordinator takes there. Also approves formulary additions, which reach P&T directly rather than through Round Table.
The pharmacy rule →The physician and APP council that sets cross-specialty standards, approves Medical Staff policies such as EMR Etiquette and order set governance, and resolves disagreements the CMIO escalates.
Order Set and OPA Governance →The radiology image viewer. At Freeman it is Change Healthcare Enterprise Viewer, opened from the Image Link icon on an imaging report or from the desktop icon. Workstations only, not phones or tablets.
PACS: Enterprise Viewer →The patient's PCP on file is where post-discharge results route.
The state record of controlled substances dispensed to a patient. Review it before signing a controlled prescription to catch a recent similar script or excessive MMEs.
EPCS: Check the PDMP →To queue an order without signing it. Students and some non-provider roles pend orders for a privileged provider to review and sign. Pended orders are not active.
The Epic environment where analysts build and where Round Table review of order sets and OPAs happens. Changes move from POC to TST to PRD.
Epic Environments →PRD is the live Epic environment with real patients. The playground is the training environment with practice patients. Haiku and Canto QR codes on this hub connect to PRD only.
Haiku and Canto QR codes →Opening a clinic visit before the patient arrives to draft the note, add diagnoses, and pend orders. Signed orders are carried out even if the patient does not come.
Outpatient Quick Start Guide →Your personal short list of favorite orders, diagnoses, and medications that appears first when you search. Built in the User Settings Lab.
User Settings Lab →Inpatient labels on the Problem List. The Principal Problem is the main reason for the stay; Hospital Problems are everything addressed during it. Both drive billing and the discharge summary.
EMR Etiquette →An order set or order group that lets nursing or other licensed staff start or adjust care under defined conditions without a provider order each time. Protocols are reviewed annually.
Order Set and OPA Governance →Letting one MyFreemanHealth user (a parent, guardian, or caregiver) see and act in another patient's chart. Converts to limited access at age 12 and ends at 18.
Epic FAQ: pediatric MyChart →The plain language summary of the visit that the patient reads. Abridge can draft it alongside your note, concise or comprehensive, from the same recorded conversation. Pull it into Patient Instructions with the .ambientptinstructions SmartPhrase.
Advanced Ambient: your note preferences →A one-click In Basket button you build yourself: a canned reply with a default subject line and message text. QuickActions do not transfer between Epic organizations, so write yours down before you leave and rebuild them here.
Recreate your QuickActions →Epic's activity for structured review, comment, approval, and build tracking of order sets and OPAs. The system of record: if it is not in Round Table, it is not getting built.
Order Set and OPA Governance →Epic's built-in secure messaging. Takes the place of Mobile Heartbeat from October 24, 2026. Attach the patient; use for non-emergent communication.
Haiku and Secure Chat Setup →Orders signed before a patient reaches the unit. They wait until the unit releases them when the patient arrives.
Inpatient Quick Start Guide →A structured Epic form that captures discrete data, such as the Patient Wishes SmartForm in the Advance Care Planning navigator.
Important Changes with Epic →A token that pulls live chart data into a note, such as today's vitals or the medication list. Refreshable SmartLinks update when copied forward; static ones do not.
EMR Etiquette: Notes →Reusable text blocks you insert with a dot phrase (for example .ros). Coming from another Epic organization, yours can travel with you through Epic Earth. Anything that does not transfer gets rebuilt in the User Settings Lab.
Epic Earth: Bring Your Settings With You →An order set for outpatient and procedural use, grouping orders, diagnoses, and follow-up. Inpatient equivalents are called Order Sets.
OPA & Order Set Requests →The family of documentation shortcuts: SmartPhrases, SmartTexts, SmartLinks, SmartLists, SmartSets, and NoteWriter macros.
User Settings Lab →Note layouts. SOAP runs Subjective, Objective, Assessment, Plan. APSO puts Assessment and Plan first so the reader sees your thinking without scrolling.
EMR Etiquette: Notes →A one-click button at the top of the Notes activity that opens a new note with the note type and template already chosen. You build your own in My Notes Settings; buttons are numbered left to right, so the first one also runs with Alt+1.
Build an Ambient speed button →The patient summary bar on the left side of every chart. Demographics, allergies, code status, care team, and key alerts at a glance.
A nightly copy of PRD that analysts use to reproduce and troubleshoot problems without touching the live chart.
Epic Environments →A trained Freeman colleague in a black Super User vest. Your first stop for at-the-elbow help.
Help during go-live →A single timeline view of a patient's vitals, labs, meds, and events for quick trending.
The Emergency Department patient board in Epic. In the ED you pick your patient from the Trackboard instead of a clinic schedule.
ED and Hospital →Everyone caring for an admitted patient, with the first contact for each team and the primary provider or team. Every admitted patient must always have a primary provider or team.
Patient Management for Providers →The Epic environment for integrated and interface testing, including EPCS and Dragon. Providers log in only when asked.
Epic Environments →A hands-on session, same day as training, to personalize preference lists, macros, and SmartPhrases.
User Settings Lab →
